Breast augmentation starts with the right choice of breast implants

Right choice of breast implants before the procedure is of paramount importance.

The most important thing is your preference regarding the size as well as the shape of the breasts you want to have. To achieve the best results, it is necessary to take into account your body characteristics (quality of the skin on the breast, amount of subcutaneous fat, your height, weight, shape of your chest wall, etc.), the current size and shape of your breasts.

During the consultation, I will advise you on the size and shape that would be most suitable to achieve a more harmonious and natural look of your whole body. You will try the test implants (sizers) of different sizes to get the best idea of how your breasts will look after the surgery.

If your breasts have lost volume and became sagged, breast augmentation is usually combined with breast lift - this is a procedure called breast augmentation with lift or augmentation mastopexy. In this way the breast can be enlarged and the shape can be further improved. The breast augmentation with lift helps your breasts look more youthful, fuller, lifted, but still natural in shape.

If you are not comfortable with the idea of silicone breast implants, you can also have your breasts augmented by injecting your own fat into the breast area, which is called fat grafting or fat transfer. This procedure is particularly suitable for women who want a moderate breast enlargement. By injecting your own fat into the breast area, the breast can be enlarged up to a maximum of one cup. Is there a part of your body where the fat is stubborn or simply too much? I can take fat from such a part with liposuction and inject it into the breast. Some of the injected fat will break down in two to three months, but after that the situation is definitive and result is stable. The procedure can be repeated to achieve an even larger breast size.

Types of breast implants

Are you worried that you won't be able to decide on the right breast implant size for your breast augmentation?

Let me reassure you, breast implants come in different sizes, dimensions and shapes. We will work together to choose the breast implant that is most suitable for your body characteristics and that will satisfy your needs. The most commonly used breast implants today are filled with silicone gel and covered with a silicone sheath, which can be either textured or smooth.

Two types of implants are used for breast augmentation:

Round (smooth or textured surface)
Teardrop or anatomical (textured surface)

The choice of breast implant partly depends on the shape of the breast you want to have after the procedure. If you want a more natural breast shape, teardrop implants are best, while round breast implants can be used to achieve a fuller upper pole of the breast. Round breast implants are more suitable for women who also want a fuller cleavage. But beware, this can quickly look unnatural if the breast implants are too big. I am especially careful in skinny women, who have smaller breasts without any fatty tissue. If there is more fatty tissue, the shape of the implants is less important.

I am often asked how long breast implants last and whether they will need to be replaced. Let me reassure you, breast implants come with a lifetime warranty, which means that if you have no problems with them, they do not need to be replaced. However, complications can occur, on average ten to fifteen years after the insertion of the breast implants, as a result of the overgrowth of the capsule (capsular contracture). If problems do occur, I recommend that you have your breast implants replaced or removed completely.

Breast augmentation has also no effect on breastfeeding or on mammography.

What are the steps of breast augmentation procedure?

Breast augmentation or boob job is a surgery that takes about an hour and a half and is performed under general anaesthesia.

Let me explain how the procedure works:

In most cases, I make a 4 - 5 cm incision below the breast (inframammary crease). I can also make an incision around the breast areola, but less often. In the case of a simultaneous breast lift, the incision is in the shape of a 'lollipop'.
I can insert breast implants above (subfascial) or below the pectoralis major muscle (sub muscular). This depends mainly on the size of your existing breasts.
The wound is closed with an absorbable suture, which means you don't have to remove it.
Special dressing tapes and compression bandage or postoperative bra is applied.
After the procedure, you will be transferred to a recovery room where you will rest for a few hours after the procedure. You are usually discharged from the hospital in the same day.

After your breast augmentation, you will receive detailed instructions about your recovery and you will be scheduled for a follow-up appointment within two or three days.

What should I expect during breast augmentation recovery?

Recovery after breast augmentation surgery is an essential part of the overall treatment process, and proper postoperative care is crucial to ensure optimal results and minimize complications.

Here is a general overview of what to expect during the recovery period after breast augmentation:

Immediate postoperative period:

After breast augmentation surgery, you will be monitored in the recovery room until you are stable.
You may experience grogginess, drowsiness, and some discomfort as the anaesthesia wears off.
It is common to have bandages or dressings over the incision sites, and a surgical bra or compression garment may be provided for support.
You will receive instructions on postoperative care, including wound care, medication management, and activity restrictions.

First few days to one week:

Pain, swelling, and bruising are common in the first few days following breast augmentation. Pain medication prescribed can help manage discomfort.
It is important to rest and avoid strenuous activities during this initial recovery period.
Wear a surgical bra to provide support and minimize swelling.
Keep incision sites clean and dry as instructed by your surgeon to prevent infection.
Attend follow-up appointments with me to monitor your progress and address any concerns.

One to two weeks:

Swelling and bruising will gradually subside, but you may still experience some discomfort.
Light activities and gentle movements are encouraged to promote circulation and prevent stiffness.
Avoid lifting heavy objects or engaging in strenuous exercise during this time.
Continue to wear a supportive bra.
You may be able to gradually transition back to your normal daily routine, depending on your healing progress.

Long-term recovery (two weeks and beyond):

Most swelling and bruising should resolve within a few weeks, but it may take several months for the final results of breast augmentation to be visible.
Follow the postoperative care instructions, including scar care techniques and follow-up appointments.
Gradually increase your physical activity and exercise routine.
Be patient with the healing process and allow your body time to adjust to the changes from breast augmentation.

It is important to communicate openly with me during the recovery period and follow the guidance to ensure a smooth and successful recovery after breast augmentation. Follow the summarized postoperative instructions for a smooth recovery:

The postoperative compression bra should be worn for six weeks.
Take your medication as prescribed.
I recommend rest for the first few days after the procedure.
Avoid raising your arms more than 90 degrees (shoulder height) and resting on your hands (pushing, pulling, lifting).
Walking around the house or taking short walks will be beneficial, but avoid major physical activity.
You can start showering 7 days after the procedure (over a waterproof dressing)
After two weeks you can start scar care to help soften and fade the scar.
You can start exercising gradually after 4 weeks, but don't forget your compression bra.
After the first few weeks, start with lower trunk exercises and avoid upper trunk exercises for 6 weeks. Running and jumping are also discouraged during this period.
You should avoid sex for 14 days after the procedure, and you should handle your breasts gently until the sixth week after the procedure.
Eat healthy foods rich in fibres, vitamins and minerals, drink plenty of fluids and get enough sleep. All these factors will boost your metabolism.

After six weeks, when the recovery is over, you can gradually start all activities. I am sure you are also wondering when the final result will be visible. The final result is visible when the tissue softens and the swelling disappears. This is usually a few months after breast augmentation.

What are the risks and potential complications of breast augmentation?

While breast augmentation is a commonly performed and generally safe surgical procedure, like any surgery, there are potential risks and complications associated with it.

It is important for individuals considering breast augmentation to be aware of these complications and discuss them thoroughly with me before undergoing the procedure. Here are some of the potential complications of breast augmentation:

Capsular contracture: capsular contracture is a common complication of breast augmentation where the scar tissue (capsule) that naturally forms around the breast implant becomes thickened and tightens around the implant. This can cause the breast to feel firm, distorted, and painful. Treatment may involve surgical intervention to remove the scar tissue.
Implant rupture or leakage: breast implants can rupture or leak over time, leading to changes in breast shape, size, and texture. Surgical removal and replacement of the implant may be necessary.
Infection: infection at the surgical site is a potential complication of breast augmentation. Symptoms may include redness, swelling, pain, fever, and discharge. Prompt treatment with antibiotics or surgical intervention may be required to address the infection.
Hematoma: a hematoma is a collection of blood that can occur following breast augmentation surgery. Symptoms may include swelling, bruising, and pain. Drainage or surgical evacuation may be necessary to remove the hematoma and prevent complications.
Seroma: a seroma is a collection of fluid that can accumulate around the breast implant after surgery. Symptoms may include swelling, discomfort, and a fluid-filled lump. Draining the seroma may be necessary to relieve symptoms and prevent complications.
Changes in sensation: some individuals may experience temporary or permanent changes in nipple sensation, breast sensitivity, or numbness after breast augmentation surgery. These changes may be temporary or persist over time.
Symmastia (uniboob): symmastia, also known as "uniboob" or "breadloafing," occurs when the breast implants are placed too close together, causing the skin to lift off the breastbone and creating a single breast appearance. Revision surgery may be required to correct this issue.
Asymmetry: uneven or asymmetrical results can occur after breast augmentation, where one breast may appear different in size, shape, or position compared to the other. Revision surgery may be necessary to achieve symmetry.
Scarring: scarring is a natural part of the healing process after breast augmentation surgery. While efforts are made to minimize scarring, some individuals may develop visible scars around the incision sites. Scar management techniques can help improve the appearance of scars over time.
Allergic reactions: In rare cases, individuals may experience allergic reactions to the breast implant materials, anaesthesia, or surgical products used during breast augmentation. Symptoms may include rash, itching, swelling, or difficulty breathing. Immediate medical attention is required in the case of an allergic reaction.
ALCL: anaplastic large cell lymphoma (ALCL) is a disease of the capsule of the breast implant. The likelihood of developing is extremely low, affecting only 0.1 to 0.3 per 100,000 women undergoing the procedure per year. It has been described in implants with a textured surface few years after the procedure. If this condition is suspected, you will always be referred for further diagnostics. If the diagnosis is confirmed, removal of the breast implant and capsule is necessary.
